Sense is a multimodal ecosystem designed to support both active and passive management of Type 2 Diabetes and its precursor Prediabetes through four interlinked, fluidly unfolding phases:
It begins with a mobile-led onboarding and a calibration phase, tailoring the system to individual characteristics, and extends into a wearable interface that enables discreet, continuous interaction with Sense' core feature and overall long-term engagement. A desktop platform complements these by connecting patients and healthcare professionals, visualising trends, and facilitating communication. All components are grounded in continuous glucose monitoring, which -though optional- enriches the experience with real-time insights for depth in care and scientific interpretation.
Together, these elements form an evolving circuit of support, seamlessly integrating personal awareness, behavioural engagement, and clinical guidance.

I. Initial Phase | Mobile-led, automatic, and observational
Bi-weekly continuous glucose monitoring is used to determine individual eating intervals and assess personal patterns of hunger and appetite regulation.
Who experiences hunger, at what time and context?
Sense knows:
hunger or habit?

II. Engagement Phase | Watch-based, mobile-led, and interactive
Watch-based micro interactions and mobile-led macro prompts function to support real-time reflection of huger and appetite, helping patients attune to internal signals and build awareness to their eating behaviour.
II.I Distinction of Hunger and Appetite
Am I hungry?
Sense catches the shift.
Before the craving does.
II.II Distinction of Hunger and Appetite
What part of me is hungry?
Sense reconnects.
What has always been linked.
II.III Intensity Assessment
How hungry am I?
And checks in.
Even when you don't.

II.IV Emotional Mapping
How am I feeling?
Sense aligns.
Bodily signals
and emotional cues.
II.V Social Determinants
What have I eaten today?
At your side.
On your schedule.
II.VI Health Literacy
What do I need to know?
Sense has answers.
When you need them most.

III. Dialogue Phase | Mobile-led, desktop-based, and interactive
Clinical communication serves as a value-creating funnel by facilitating a structured dialogue between physician and patient grounded in visualised data, thus fostering a strong doctor-patient relationship.
What data was collected, and how should it be interpreted?
Sense spots it.
Patterns. Habits. Clues.
And has answers.
When you need them most.
Sense joins the dots. You and your medical team.

IV. Maintenance Phase | Watch-based
Essential micro-interventions introduced in Phase II will undergo continuous iteration and reinforcement to promote long-term behavioural adaption and sustainability.
What has changed?
Sense makes sense of experience.
You make better choices.

Prospects | Expanding use cases
With its multimodal approach, Sense closes a critical gap in managing lifestyle-driven chronic diseases. By integrating patients daily lives, experiences and emotional realities, it transforms overlooked insights into actionable knowledge, bridging unmet needs, easing clinical strain, and enabling more targeted interventions. This clarity helps untangle complex metabolic imbalances while revealing individual levers for tailored care. Beyond metabolic health, the Sense approach stands as a model for other fields where patient-centred care and holistic perspectives remain lacking.
